To submit a routine service request, simply complete the open fields in the form below. The items marked with an asterisk are required for the form to be submitted to our Service Managers.

If you have an Emergency Service Request, please call (765) 490-6082. If no one is available to take your call, please leave a message with you name, phone number, apartment number and description of problem on voice mail. A manager will call you back promptly.

Priority (Emergency) Service Requests include:
    • No Electricity throughout the apartment
    • No Heat in temperatures below 55°F
    • No Air Conditioning in temperatures over 88°F
    • No Water throughout the apartment
    • Water flooding into the apartment (if possible please shut off the valve to stop the flooding)
    • Smoke Alarms not working properly
    • Situations where access to the apartment may be compromised (i.e. broken windows or broken        locks).
